Insurance
We are Medicare Participating Physicians and members of HMOs/PPOs. If you are a member of an HMO or PPO, please check your plan to see if you need a referral prior to making an appointment. If you have insurance, including Medicare, BAIPA, or Public Aid, please have your card available so we can copy it for our records.
If you have an HMO or PPO that our physicians are contracted with, we will bill them directly. However, you must provide us with proper ID cards and inform us that your insurance coverage is with one of these contracted services. We are Medicare Participating Physicians. We will bill Medicare directly for our senior citizen patients. If you have Medicaid (Public Aid), you must present your card each time you are seen; failure to do so may result in your being responsible for your charges.
For those patients with commercial insurance coming in for office visits, lab or routine exams, you will be given a copy of your charge slip to submit to your insurance company. This is a superbill that contains itemized charges, date of service and diagnosis. Complete your portion of your insurance claim from and attach your copy of the superbill and submit to your insurance company. We do not submit insurance for these services. Surgical patients should furnish us with insurance claim forms as soon as they are notified that they are scheduled.
We will be happy to help you receive maximum insurance benefits; however, the agreement of the insurance company to pay medical benefits is a contract between you and your insurance company. Responsibility for payment is your obligation, regardless of insurance coverage. Payment for services is requested if your insurance has not responded within 45 days of the date of service. Any over-payment will be refunded. If you have any questions, please call our Bookkeeping-Insurance office between 8 a.m. and Noon Monday through Friday.
