Insurance 
Our group participates in most insurance plans and PPO’s, e.g. BC/BS, Humana, ECOH, Personal Care, Medicare, Illinois Public Aid, and many others.

If you have an insurance plan that our physicians are contracted with, we will bill them directly. Please have your insurance card available so we can scan it for your claim and our records. For those patients with commercial insurance we will submit your claim to the insurance company. Patients are responsible for their portion, such as co-pays or deductibles.
